(You can only pick up one, because they start a new quest, but they are dropped randomly by many of the opponents so that everyone can get one.)
While in the Uldaman Cave or Dungeon Instance, (s)he most likely picked up "The Shattered Necklace" and kept playing. I think this may have occurred while the person was trying to turn in one of several quests that were completed in Uldaman, specifically, "The Platinum Discs" which you complete at the end of the Dungeon after you kill Boss Archaedas.
